A practical medical-record checklist
A complete, well-organized record package can shorten avoidable back-and-forth before specialist review.

Core documents
- A concise medical summary with dates
- Pathology reports and, when requested, slides or blocks
- Imaging reports plus original DICOM files
- Laboratory results, medication list and allergy history
- Prior treatment and operation notes
Remove duplicate pages, label every file clearly and keep the original language version alongside any translation.
What to do next
For a conversation about your situation, contact a coordinator through the consultation form. Do not send medical records through the public form.
